Leonard Sellem bc7eef88bf Note the Hypermnesic tool surface in the entry 
Cite the 7 MCP tools (search, read_note, build_context, resolve, think,
list_folders, and the gated commit_note write), matching how neighbouring
entries describe their tool counts. read_note and per-parameter schema
descriptions just landed, lifting the Glama Tool Definition Quality score to A.

Leonard Sellem 1c271c71b9 Fix Glama badge URL for the Hypermnesic entry 
The earlier badge used .../leonardsellem/hypermnesic/badge.svg, which 404s.
Switch to the canonical Glama score badge (.../ak6x81u3rr/badges/score.svg) —
the small inline format every other entry in this list uses — now that the
server has a published build and a live score.
