Add mcp-gatehouse

Permission tiers, approval gates, and secret-redacting audit logging for
MCP servers, enforced inside the server at the tool boundary.

Includes the Glama score badge, per the listing requirements. Rebuilt on
current main; the previous branch was ~2500 commits behind.
